When completing the form please let us now if it is urgent and the nature of your problem if you feel comfortable doing so. We may ask you to see the practice nurse if it is more appropriate to do so, this is the case for contraceptive reviews and some injections such as B12 and depo. Remember our practice nurse also runs the chronic disease clinics for asthma, diabetes and chronic obstructive pulmonary disease (COPD- respiratory disease)
